Exhibits Request Form
This form is for exhibit related request ONLY.
Instructions (PLEASE READ)
If you are in an exhibit and notice a broken item, please report it here. If you are looking to have an item created by the exhibit’s team for a program or event, please submit them here.
Examples of Broken Exhibits Items:
- Torn Graphics
- Broken Cash Registers
- Ripped Upholstery on Seating
- Flat Tires on the VW Bug
- Broken Rope on the Nile Pull
Plan Ahead. Timing will depend on the request made. We highly recommend the following timeframes:
- 4 Months in Advance: For large built created items for programs. For long term repairs to exhibits that will need supplies purchased or items that will require an outside vendor. Example: replacing the tops of Dune Walls in Tot Trails or any re-upholstery jobs.
- 2 Months in Advance: For smaller built created items for programs. For smaller not urgent repairs to exhibits that we have supplies on hand for but need to work into the existing exhibits project schedule.
If there is an emergency – Such as the broken item will cause harm, please DO NOT use this form and instead email David dberman@portdiscovery.org AND Hope hmyers@portdiscovery.org.
Please fill out a separate form per request. So, if you have one broken exhibit item to report, fill out one form. If you have one exhibit request to be created, and one broken exhibit item to be reported, please fill out a separate form for each.
There should be one contact person per request.
This is the person that we will communicate with regarding the request. This person will have the responsibility of sharing information with any other team members and of collecting/organizing all feedback and supplying back to the exhibits contact working on your project.

Facilities Request Form
This form is for Facilities related request ONLY.
Instructions (PLEASE READ)
If you are around the building and notice a broken item, please report it here. If you are looking to have a wall patched or painted by the facilities team, please submit them here.
Examples of Broken Facilities Items:
- Leaking Faucet
- Light Out
- Chunk of Wall Missing
- Damaged Carpet
- Broken Ceiling Tile
Plan Ahead. Timing will depend on the request made. We highly recommend the following timeframes:
- 4 Months in Advance: For large painting jobs and long-term repairs to the building that will need supplies purchased or items that will require an outside vendor.
- 2 Months in Advance: For smaller painting jobs and smaller not urgent repairs to the building that we have supplies on hand for but need to work into the existing facilities project schedule.

Miscellaneous Request Form
This form is for Miscellaneous Exhibits/Facility Requests.
Instructions (PLEASE READ)
These are requests that do not fall under a specific department’s jurisdiction and do not fit under the above forms’ qualifications.
Examples of Miscellaneous Requests:
- Printer Issues/ Ink Replacement
- Furniture Building/ Installation
- Moving/ Removing a Large Item
- Large TV/ Laptop/ Podium Requests
- Pipe and Drape Requests
Plan Ahead. Timing will depend on the request made, however, for smaller requests, we recommend submitting the request a minimum of one week in advance. For larger, please submit them as soon as possible and at least two weeks in advance.
If there is an emergency – Such as the event you are having is tomorrow, please DO NOT use this form and instead email David dberman@portdiscovery.org AND Hope hmyers@portdiscovery.org.
Please fill out a separate form per page. So, if you have miscellaneous request to report, fill out one form. If you have one printer issue report, and one moving item to request, please fill out a separate form for each.
